ON AN ESTABLISHED TEMPLATE AND PROCEDURES DEVELOPED FOR OTHER FORHP GRANTEES,
THE TAAP WILL INCLUDE BACKGROUND INFORMATION ON THE GRANTEE PROGRAM AND
NETWORK, PERCEIVED STRENGTHS AND POTENTIAL BARRIERS, GRANTEE NEEDS, TA STRATEGIES,
EXPECTED OUTCOMES, AND TIMELINE. IN ADDITION, ESTABLISH THE ROUTINE MONTHLY CALL
TIME AND DOCUMENT IT IN THE TAAP.
